Subscribers personally claiming their graduation photo packages must bring a VALID IDENTIFICATION CARD.

Authorized representatives must bring the following:

☑️ Signed authorization letter from the subscriber

☑️ Photocopy of the authorized representative’s ID and the subscriber’s ID

 

Other office transactions such as claiming of yearbooks are entertained at GMH110 from Mondays to Fridays, from 9:00 am to 12:00 pm and 1:00 pm to 4:00 pm, and on Saturdays from 9:00 am to 12:00 pm.
